New Students
For new students, below is a quick checklist of your immunization requirements that you should provide documentation to File MD at least a month before the first day of classes:
- MMR (Measles, Mumps, Rubella)
- Chickenpox (Varicella)
- Tdap (Tetanus, Diptheria, and Pertussis)
- Hep B, 3 shots (Hepatitis B, completed series)
- Tb (Tuberculosis skin test)
- Flu (Influenza Vaccine)
- Yearly Physical Exam
- CPR (For health care professionals)
