Appendix B: Program Manual Attestation

The students in the Post-Professional OTD Program are responsible for reading and understanding the policies and procedures that reflect the purposes and requirements of the academic and fieldwork programs at the MGH Institute of Health Professions, as provided in the Institute Catalog and the PP-OTD Program Manual.

 

The statements contained within the PP-OTD Program Manual serve as a supplement to the Institute Catalog, delineating policies and procedures that are specific to the Post-Professional OTD Program.

 

Students will be expected to attest to knowing and agree to abide by the policies and procedures contained within the Institute Catalog and the Program Manual for the Post-Professional OTD Program.  Any questions regarding policies and procedures should be directed to the Program Director.

 

 

I understand that I am responsible for knowing and abiding by the policies and procedures contained within the Institute Catalog and the Program Manual for the Post-Professional OTD Program.  All questions have been answered to my satisfaction. I understand these policies are subject to revision, and that I will receive due notice of any changes that are relevant to my status in the program.
